To receive an entry by mail, on a plain piece of 3 x 5 paper, hand print the following required information: first and last name, address, city, state, zip code, and phone number. Email address is optional. You must also confirm that you are 18 years of age or older, by writing the words I confirm I am 18 years of age or older on 3 x 5 paper entry. No mechanically reproduced entries accepted. Mail entry in a hand-addressed, business-size (#10) envelope with first-class postage affixed to: Andy's Frozen Custard, Attn: Cariel Grove, 211 E Water St. Springfield, MO 65806. Mail-in entries must be received each Tuesday 9/17/24 - 12/3/24.
